a religious trust is not registered under section 12aa of the income tax act. whether such trust is liable to audit?? which itr will be required to file the return of such trust??

We have purchased Easy Recharge from Reliance communication of 2,79,00,000 Rs they have given bill of Rs 2,90,00,000 but they have given TDS on 26 lakh rs of 2,60,000rs.
In this 26 lakh rs they have given 10 lakh rs directly to retailers(our customer,reliance is our supplier). Remaining amount to us
Now we want to claim TDS on 26 lakhs

One of my client alter the name of company in July 2016 now the problem is in audit report & director report what name will appear.In ROC return new name of company is displaying.

as per guidance note on fixed assets register if the company has not maintained fixed asset register what will be its effect on auditors opinion?
do we need to qualify ?
or unfavourable opinion is alright?
