Revenue Audit Report

Dear Experts Im doing revenue Audit Report for an health care BPO company. I have done the auditing and found lot of discrepancies. I would like to present the audit report to their management. kindly help me in this regard and share me the revenue Audit Report format as soon as possible Thanks in advance.

Tds

We have purchased Easy Recharge from Reliance communication of 2,79,00,000 Rs they have given bill of Rs 2,90,00,000 but they have given TDS on 26 lakh rs of 2,60,000rs.
In this 26 lakh rs they have given 10 lakh rs directly to retailers(our customer,reliance is our supplier). Remaining amount to us
Now we want to claim TDS on 26 lakhs
How to show it in P & L??
How to claim it??

Is it Complusory to report on all the 16 clauses of CARO 2016, ie If a clause is not applicable to a Company, can we just skip it off or, we have to state the caluse and mention there as Not Applicable

Name of company changed after closing of F/Y

One of my client alter the name of company in July 2016 now the problem is in audit report & director report what name will appear.In ROC return new name of company is displaying.

as per guidance note on fixed assets register if the company has not maintained fixed asset register what will be its effect on auditors opinion?
do we need to qualify ?
or unfavourable opinion is alright?
