then employer & employee contribution to PF 12% on 5000/- i.e. 600/-

my question is if the professional tax will be calculated on Rs. 5000/- or 5000/-plus employer contribution i.e. Rs. 5600/-

I have completed one year of my articleship .I wanna take transfer from my present principal due to some reasons. Can i get transfer on medical grounds. Did I have to submit any proof for medical reasons . If yes, then what kind of proof ??? I am really tensed someone told me its almost impossible to take transfer.Does institute reject transfer request??? Pls HELP.
