File for change in pan with any date in it.
You will get an error from NSDL stating the date on PAN.

In this case it is better to contact NSDL and get the DOI of your PAN and to know the procedure to get reprint.

If you have not orignial DOI, then you have to fill date on which bank account is open for huf.
because in case of correction of DOI, they demand proof regarding such correction, then now you attached copy of bank statement in which account open date is given.
