I have incorporated a company now i have to apply for PAN card what all documents do i need to submit with the application form

You will have to file form 49A. You can file it online from below link and also check out the instructions--

You will have to submit "Copy of Certificate of Registration issued by the Registrar of Companies." along with form 49A.
