26 May 2011 since the guardian had signed at the time of application for PAN now minor became major.
for changing sign which form applicable............

26 May 2011 You need to fill up a form called "Request for new PAN card or/and changes or correction in PAN data" which is ment for corrections of data in PAN card and issue a fresh PAN card after corrections with the old PAN card number.

You need to spend Rs.5 for form and Rs.67 as fee for a new card. It will take about 15 to 20 days to obtain a fresh card if the documents submitted by you is correct. You can see the FAQs on PAN data correction in NSDL website.
