Whether assesse can claim the deduction of sec 80U after 4 A.Years (in Case of permanant Disability)

Yes if u fufill all other conditions u/s80U please note there is no time limit this can be claimed every year.

You can Claim U/s 80U in Every year. upto 75000/- is Permanat Disablity. but you just have to do the attachemnt as per perscribe formats.
