Dear sir,how to cancellation of din and which procedure follow? which documents are submitted?

Please e-file form RD-1.
Attachments are -
a) declaration in Rs. 25 non judicial stamp paper that DIN has never been used and he/she has never been appointed as director in any company.
b) form DIR-5
c) identity proof like PAN card/voter ID/passport/driving license and
d)proof of residence like electricity bill/telephone bill/bank a/c statement/ driving license/voter ID/passport.
