What if accountant makes mistake?

I am a freelance software developer. I have consulted a CA for filing my IT return. He prepares P&L account, capital account, balance sheet and then calculates the tax. He files the ITR4 and ITR-V. The problem is CA doesn't do anything but his office staffs do everything and they make mistakes almost every time. My question is -
1.) If there is any mistake by them in ITR4, P&L etc, should the accountant be responsible or me as I am signing the ITR-V?
2.) If they aren't responsible what should I do, as I don't know anything about accountancy and ITR4 - there are lot of fields! (should I tell them to attested or something like that? )
3.) I am self-employed and my income is 8.5 lacs, making P&L, balance sheet etc. and filing ITR4 is all it needs? or I have to pay any TDS, service or any other tax?

you have income more than exemption limit..visit trusted ca in your locality and tell him all ur particulars, he definitely solve ur issue...
