Can you please tell me who decides the break up of Salary. Is the Employer or Employee?

it depends, if salary break up of the employer is in place, then it would be applicable for all the employees and then you have to follow, and;
if not, then you can make it to suit you to minimise the tax liability...

IT DEPENDS ON COMPANY'S POLICY.
IF THERE IS ANY POLICY TO DETERMINE SALARY BREAKUP OF ALL EMPLOYEE , THEN YOU HAVE TO FOLLOW IT.
IF COMPANY ALLOW TO MAKE SALARY BREAK UP BY EMPLOYEE HIMSELF ,THEN HE CAN MAKE IT.
