If Assesse is providing morethan one services from different Premises so:-
(I)he should go for seperate -2- registation or single registration as a centralize registration and
he need to file one or more Service tax returns according to services.

service tax return has two major division for return purpose, a centralized registration cover all places provided these are included in registration.
1) provider of taxable services
2) receipent of taxable services

you need to apply for registration for service tax showing both the services.
