29 September 2015 Yes it is compulsory to submit hard copy in duplicate to the department.

29 September 2015 Do comply & give all the details & Take sign & stamp of the person receiving the same. then its there problem not yours.

But yeah make sure you fill all the details requiredin from 27 C.
