11 December 2015 Normally Address proof, ID proof, for the place of Business, proprietor, partners and their Photos etc as the case me. Online and manual application forms are under process for this. But state to state this may differ. Please mention your state to give correct views from the experts. Consult a local Auditor in this regard.
