i had applied for CA final Nov. 2011 attempt provisionally.. I want to know upto which i have to submit certificate of service with ICAI and what is the standard format...

You are required to submit Certificate of service latest by 30th nov-2011 in standard format recived by you along with letter of provisional appearance in examination

I had received only admit card & no letter of provisional appearance. so in which format should i send letter to the institute.

you are not required to send letter of service,if no letter of provisional appearance in examination recieved by you.
