Reimbursement of medical exp. rs. 15000/-

As per IT Provision an exemption up to Rs. 15000/- is allowed while computing taxable income.Pls suggest what should we do for allowing deduction of Rs. 15000/- in form 16
1)We should obtain Medical bill of Rs 15k. or
2)we can just obtain a dec. from employee that he has incurred Med. Exp of Rs. 15k & produce the documents as & when ask by company. OR
3)Simply we can allow deduction of Rs. 15k without asking anything from employee.
pls suggest 1)what is best practice 2)Not best but can be accepted.
