09 May 2017 Again purchase form booklet from ICAI & ID Card if they give it individually. Because ID Card is compulsory. On getting registered that ID Card will return to u& you have to submit it on completion of your articleship.

else you can ask your employer to give you a letter in favour of ICAI, to issue you a physical form 108
