Procedure to file RTI for Tax Refund

I had filed return for AY 2008-09 last year and I am still awaiting my Tax Refund. Can anyone help me in filing RTI for Refund and Sample forms and Letter that is needed to file the RTI.

You have to meet with Income tax officer directly with copy of acknowledgement of RTI.
