What documents an employer can insist fior disbursal of medical expenses? A Doctor's letter giving certificate that the patient was under histreatment and had incurred a specified sum is acceptable?

Certificate not acceptable bills for purchase of medicine, consultation, lab fees and hospital charges are required.

In my case The Dr is a registered medical practioner and he is treating the employee on regular basis and he gives his own medicines and write the amount incurred and give a letter.
