What is form for reporting the provision of Bad Debts details to Foreign Exchange Authorised Dealer (ie, our Bank)

To the best of my knowledge, there is no specific form. All AD have their own formats. Or else, you may simply issue a credit note to the debtor and give a copy to the AD
