sir i needed to change my name and address in icai due to some mistakes made leaving certificate and the same name is entered in icai so, i needed to change my name and address in icai so how to change can anybody give suggestions

On submission of request Letter duly signed by the student along with the respective documents as stated below:
For Female students on account of Marriage For Male/Female students
On submission of request Letter duly signed by the student along with a copy of marriage invitation card or attested copy of marriage registration certificate. Attested copy of the 12th/Degree Mark sheet
Affidavit undertaken before notary public or a first class magistrate Attested copy of the Gazette Notification
Change in Address
On submission of request Letter duly signed by the student.
Note:(Student Registration Number to be stated in all communications)
