Stamp duty on for incorporation of company

I want to know whether DIR 2 and INC 8 to be taken on stamp paper if yes then what is stamp duty to be paid for gujarat state?
I want to know that bank passbook will be treated as address proof u/r 16 of companies incorporation rule 2014
Further please provide me that whether subscription of shares has to be written in own handwritting of promoters . please give me reference the same in act or rules.
whether noc from owner is required if rent agreement is provided in which its clearly written the same can be used for the business purpose.

Date of issue of share certificate

Can anyone please tell me what should be the date of issue of share certificate on the face of it? can it be date of allotment or it has to be date of actual issue of share certificates?
