In tax Audit i Already add CA, but Now it is shown as Inactive. Sir , How can convert CA Status From Inactive to Active.

Please guide me in the following issue.
I did a tax audit u/s44AB of I-Tax.
Date of Audit-08/09/2014.
Uploaded form 3CD-3CB,P/L and B/S.
Before Approval by the assessee some mistakes found in P/L,i.e some amount of depreciation wrongly transferred to Misc.expenses A/c.
Assessee rejects the form.
Net profit,B/S figures are not affected.Correct amount of depreciation reflected in form 3CD.So no change required in 3CB-3CD form.
Now P/L is rectified and reasons of rejection given by the assessee.
Now I have to re-upload the form again after correcting P/L only.
My query is:-
1.Whether date of audit should be changed(date of rectification of P/L) or not (previous date of audit was 08/09/2014)
2.What option to be chosen in Filing Type,i.e.
ORIGINAL
REVISED
OTHERS(with reason)
Please guide me.I have to re-upload the form.
