pls provide me the proper formet of ca witness in moa and aoa as per companies act 2013.

Witness to the MOA and AOA has to write above his/her name and address the following sentences.
"I witness the above .... subscribers who have subscribed and signed in my presence on the ...day of .....2014 at...... Further, I have verified their identity details for their identification and satisfied myself of their identification particulars as filled in ".
