my friend lost his pan card last 3 months back. what is the procedure for applying for duplicate.

Hi Yugandar,
Tell him to fill the PAN data change request application form and submit with requisite fee to the NSDL.

Just fill the form and don't tick in left side as you don't want to make any changes in the data.

Obtain form 49A. Fill up details exactly as in the details furnished in lost card application form. Attach address proof, identity proof, photograph and submit either in NSDL or UTI counter with fees Rs.67/-
NB:- Rule says FIR from Police station is to be obtained and attached with form 49A. But now it is not insisting.

Please fill the 49A form and submit it with document for proof identity, residence and photograph with NSDL centre in your city.
