Dear sir,
one of my client who has make his pan card from me , but however i came to know after one year that there is an mistake in pan card - the mistake is that in pan card the name of the client is correct but on site it show wrong name and on that wrong name pan card number alloted to the client .Therefore what should i do in order to change the name in the site as well as wrong pan no ?

file form 49A (correction Form)
with a photograph and correct proof of his name for the same PAN number
you will get the duplicate PAN CARD
