NON submission of form 108 within stipulated time

my articleship was completed on 1st april 2011 for the period of 3yrs. but was unable to deposit it within time due to examination in may 2011. so want to know the format for the application for request for condonation.

Just write a letter to the Institute asking for condonation of delay giving reasons. No separate format is prescribed.
