Non credit of tds on 26as

deductor has deduct tax from deductee in f.y.11-12 & paid to gov.and also file tds return but tax credit not seen on 26AS.
also tds return reject by nsdl.
how to resolve the matter? & how to claim for it?

deductor may not have quoted correct pan of deductee or may hav made similar mistake.
