You are required to apply and submit a written request to the concerned Decentralised Office for obtaining duplicate membership ertificate/certificate of practice as an Associate or a Fellow member as the case may be.
You required to submit an affidavit in the prescribed format duly sworn-in before a Notary/First Class Magistrate or an affidavit on a plain paper and get the same attested from the authorized office of Indian Embassy/Consular General's Office.

The fees is Rs 500
