What if one lost his original ca membership certificate, what is the procedure to get the new one issued from the ICAI ??

09 August 2014 You are required to apply and submit a written request to the concerned Decentralised Office for obtaining duplicate membership ertificate/certificate of practice as an Associate or a Fellow member as the case may be.

You required to submit an affidavit in the prescribed format duly sworn-in before a Notary/First Class Magistrate or an affidavit on a plain paper and get the same attested from the authorized office of Indian Embassy/Consular General's Office.

The fees is Rs 500
