I required indemnity bond format for misplace the original ST- 2 certificate.

This deed of indemnity made on this the 1st day of
January,2015 By YASH KHATRI son of Mr. HARI KHATRI resident of Flat no.1 ,Club Apartments, M.G.ROAD,
Bangalore -560001 carrying on the service of WORKS CONTRACT at Bangalore with registered office at Shop No.4, Lulu Mall, M.G.road,
Bangalore 560001(hereinafter called the
indemnifier which expression shall unless excluded by or repugnant to the context be
deemed to include his heirs, administrator, legal representative, successors and permitted assigns)
Whereas on the 1st day of April 2012 ,service tax registration had been granted
in favour of the indemnifier vide ST-2 bearing no ACPPR6666HSD001.
AND WHEREAS the indemnifier has represented to the Government that the said service tax registration certificate has been misplaced and irrecoverably lost from him.
AND WHEREAS at the request the indemnifier the Government has agreed to issue a duplicate service tax registration
certificate upon the indemnifier giving such indemnity as hereinafter contained.
NOW IT IS HEREBY AGREED by the indemnifier that:
In consideration of the said premises and of the agreement on part of the
Government in issuing a duplicate service tax registration certificate ,the indemnifier will indemnify the Government and keep the Government harmless
from and aganist any loss that my be caused and all expenses which may be incurred by the Government in relation thereto or in connection therewith.
In WITNESS WHEREOF the indemnifier hereto have set and subscribed his hands
hereunto on the day and year first above written.
Signed by the said indemnifier in the presence of the following witnessess.
