Format of income tax refund request

Can anybody provide me format of income tax refund application for income tax refund as I want to file income tax refund application of my father to get refund of a.s.2004-05. and how can i get income tax refund means procedure for getting income tax refund.

28 March 2012 You have to file a return to claim refund of Income Tax.

If you have filed a return and have not received the refund till date, then write an application to the Assessing Officer.

Give details of the return and attach a copy of the acknowledgement of the return.
