Can any one Provide me the format of "Affidavit and declaration by first subscriber and director" for Company Formation.

Pl execute in stamp paper and notarize.
I, ............ , aged about 30 years, residing at house no. XX/-514 , CA Nagar , Kochi , Kerala-682016 solemnly affirm and declare as follows:-
THAT I am the first Director of ..................... PRIVATE LIMITED, a company proposed to be incorporated under the Companies Act,2013.
THAT I am conversant with the facts of the affidavit and competent to make this declaration.
THAT I am not disqualified as per section 164 of the Companies Act, 2013 from appointing as a Director in ............................... PRIVATE LIMITED.
THAT I have not been declared as proclaimed offender by the Economic Offence Court or Judicial Magistrate Court or High Court or any other Court.
I, .............................. , the above deponent, do hereby declare and verify that the contents of paragraph 1,2,3 and 4 of this affidavit are true to my own knowledge and nothing material has been concealed and no part of it is false.
