Disclosure of exempt income in itr

During 2011-12 (F/Y) My friend received PF, Gratuity, earned Leave etc on his retirement, which is exempt from tax-
Now my question is whether he should disclose all these income on consolidation basis on exempt income box.
.
Also take care of item no 2 in Schedule S, Details of Income from Salary.

In given case i will file ITR 1 not ITR 4 and in ITR 1 their is only on place having name " D 19 Exempt income only for reporting purposes (form dividends, Agriculture income < 5000)"
