06 October 2010 For DIN application my father name is single (dharambir singh) for that i have givin a affidavit where my grandfather name added my father last name. how i will fill my father name in din application as in my pan card or as in affidavit
suggest me
in pan card (dharambir singh)
in affidavit (dharambir singh randhir singh)

In General, Din requires:

Name: Dharambir
Middle Name:.........
Last Name: Singh

In this format make affidavit and provide to DIN Cell.

singh is not my father last name its a middle name
for that i have gave affidavit and fillled the din from as
father name first name dharambir singh
last name randhir singh
(grantfather name)
now it again rejected due ti miss match of name filled and evident attached..
i have mention my name as
first name ---- jyoti
last name dharambir singh

plz suggest me how i fill form this time
