12 January 2013 Kindly guide me what to do if AGM date is wrongly mentioned due to similarities in the name of the company in Form 23B and whether it can be revised or not ...

Is there any requirement to attach declaration and if so , please provide me the format of the same.

You can not file revised Form-23b with ROC. You should file a fresh Form-23B with ROC with correction.
