Hello,
I'm converting a Pvt Ltd into a LLP and I'm filing name approval form. My query is which of the following options to select in Form 1:
1. Incorporating a new Limited Liability Partnership (LLP)
2. Changing the name of an existing Limited Liability Partnership (LLP)

I think you've misunderstood Let me elaborate.
I have a Pvt Ltd Company(which has its CIN no. but not a LLPin) say named Yash Pvt Ltd. I want to convert that Pvt Ltd Company into a LLP say named Yash LLP. Now the first step is to find whether the name 'Yash LLP' is available hence I'm filing Form 1 for Name Approval.
Now that I don't have a LLP how will I write a LLP Identification no.?

You are conversing the private company in to LLP, so point no.6 of Form-1 is applicable for you and not point no.9 will apply on you.
So no need to mentioned LLPIN.
Point 9 apply for change of name.
