Change of name in pan card

06 May 2011 Hi experts,

I am having a PAN card of my own.Actually in that i had given the wrong sur name.I want to change the sur name now.So i am planning to apply through the " CHANGE REQUEST FORM". But i have one doubt.My full name is "Aneesh Kumar PK". the PK is my sur name.It Can be abbreviated as "Palarivaalan Kodoor". I want to give like "Aneesh Kumar PK" in my new pan card.Is it possible?Can we give initials as sur name?Is it necessary to expand the initials.

Then is it necessary to enclose the original pan card along with the application or just a photo copy is enough?

Experts,Please give the suggestions.

06 May 2011 you can give intial of your surname provided your proof of identity has the same intial. you need to enclose original pan card with application.

18 May 2011 Dear Friend, Correction in PAN card is possible in every manner, ensure that you should have proper documents in your hand. IF you have proper documents stating that your surname and name as you want then you can fill PAN correction form.
