Change of address in PT

Please suggest . If we want to change our register address in Professional Tax Certificates due to office shifted from one place to another within city . How to change address ?

Need to file written application to your Jurisdiction PT officer, to transfer the registration no. to different ward. Attach a copy of proof for new address .
