Sir, i didn't surrendered my old id card during termination and on re-registeration .. plz any one suggest me what to do?

I have joined articles on February 25,2013. On 16 July,2013, i applied for termination. I Have posted form no. 109 signed by the principal.Even after 30 days i didn't got any response from the ICAI. regarding my termination of my articleship. Should i wait for some more time sir?

SIR, IHAVE SENT MY FORM NO.109 TO INSTITUTE FOR TERMINATION.I DIDNT SURRENDERED MY OLD ID

sir,i have applied for termination and sent form no.109 to icai,but i didnt got reply from icai since one month.should i wait for some more days? and i didnt snt my old id card to icai along with 109 form.should i have to send ?i joined artcls on feb 25 .plz sugest me sir

IN FORM ALL THE DETAIL ARE FILL IN BELOW THE IS OPTION IS "SAVE" AND "CREATE FILE " I PRESS THE BUTON IN CREATE FILE THE DILOG BOX IS OPEN THERE IS THREE OPTION

1) INPUT FILE NAME WITH PATH FOR CSI FILE
2) CONSOLIDATED FILE PATH
3) ERROR/ UPLOD STATISTIC REPORT FILE PATH IN THIS OPTION WHICH FILE IS BROWSE

Can anyone please tell me how to get converted from ipcc to intermediate(ipcc).
I went through conversion form but there is no point discussed for conversion from ipcc to intermediate(ipcc).
pls guide me on this.
