27 April 2009 One business is closed down. Its original service tax registration certificate is lost. To cancel the service tax registration original certificate is required or an affidavit is required. Does any one have an affidavit. Pls help. Thanks in advance

In case of lost certificate,the assessee can apply for a duplicate registration certificate to the jurisdictional Range Superintendent after depositing a nominal fees through a TR-6 challan.
