1.MOA & AOA in case of company and Partnership deed in case of partnership

2. ID & Address Proofs of business and propretiers or partners or directors

3.place of business details

4. turnover details

5. materials delaing details

6.prescriber registration form along with fees and security deposit

7.Local authority licence
