Vat charged on material amount including packing and insura

Kindly let me know whether VAT is levied on basic amoount of material plus packing and insurance. If VAT is leveied on amount including packing and insurance, please provide the specific circular or notificaiton .

VAT is charged only sale amount of material. It did not contain packing and insurance charges.
