Termination of articleship-documents to be sent to institute

27 May 2015 I am in my second year articleship, my mother is suffering with back pain and is a diabetic patient. For her further treatment I will have to accompany her to Karla and stay with her till she gets better.(I am a resident of Bangalore).

Please tell me what all documents are to be sent to the institute seeking termination. How long will it take to get the institutes approval?
