Termination of articleship

My 3 and 1/2 yrs of articleship will be completed on 02.07.2010 and final exam is due in Nov,2010. After completion of articleship which forms or documents are to be signed by MIT & me and submitted to ICAI ?
Also does institute sends any certicate or acknowledgement ?

You need to send form 108 only to the institute. you wil get mail that ur articleship is over and terminated. nothing more.
