Required format and content to prepare paper book

I have to prepare the paper book submit to cit appeal. the matter related to disallownace of employees contribution to pf & esi, if late deposit but deposit beor filing of return of income.., please share any format or guide how to prepare the same..

27 May 2017 1. Prepare index on the first page of the paper book mentioning the case number, case title, assessment year of appeal and concerned CIT(A). For example, BEFORE THE HONORABLE CIT(A),..... IN THE MATTER OF........... AY.........
2. Prepare index in tabular form i.e. s no, particulars, page numbers. You may also mentioned ground number.
3. Then annex relevant documents in serial number mentioned in index and each page of paper book consecutively numbered and certified wherever necessary.
4. On index page, also mention a brief statement in certificate form that "Certified that documents at s no ...... to ........... were before the Learned AO at the assessment stage."
5. Thereafter it should be signed by the AR.

29 May 2017 There is no specfic format other than the above but refer the relevant page number of paper book while preparing submissions.
