Request for supply of certified copies of answer books

30 July 2012 I want to apply for Supply of Certified Copies o Answer Books of CA Final May 2012 through online.It is asking to upload 'Handwritten Request" for the same.What does it actually mean?
Is it just filling up Printed Prescribed Form?
It also gave a prescribed format along with declaration.If I scan those 2 pages then I would be with 2 files whereas I can upload only one file.
Please clarify me at earliest.

31 July 2012 You have to Upload only the Declaration (Filled up with your handwriting) Scanned as per the specifications given (A4, 200dpi, upto 200Kb)
