07 January 2010 our client is a private ltd co. It requires registration of service Tax. What are the enclosures to be made with Form ST-1? We were informed that a boards resolution for authorisation of one of the director for signing ST-1 is required. Is it necesary?

1) Copy of PAN card.
2)Copy of MOA, AOA and certificate of incorporation.
3)List of directors.
4)Power of attorney/ Board resolution authorizing the signatory.
5)Breif details of services provided.
6) Date of commencement.
