Regarding articleship extention

19 August 2015 I registered myself as an article assistant on 16-08-2012 and took transfer on 10-08-2013 and was required to complete my remaining period of articleship i.e 2Years 05 Days under another CA.
I registered under another CA on 17-08-2013 and so now my articleship is to be completed on 21-08-2015.
but my CA extended my articleship till 21-10-2015.
Now i want to know that which form should i file with icai and till what date.?
and if i have to file form 107 then what should i attach with it.?

19 August 2015 What letter you got from ICAI when you sent the transfer form along with new 103?
Stick to those dates mentioned in the letter.

10 October 2015 I got a later Stating that my articleship is extended for 35days.
which is From 22nd August to 26th September.
Now I have to submit form 107 and form 108.
But where should i get the sign of my CA,Witnesses and mine bc there are 9 boxes like Signed Sealed and delivered in presence of
