Publication of Surname Change in Gazette Notification

Dear Sir,
I applied for change of Surname of a client in PAN card from Mittal to Agarwal. But the pan centre at mumbai is asking for some Gazettee Notification in this respect. Can somebody help me what type of notification gazettee is there and where do we get it from. Here at Nagalnd , I enquired from court thru advocates but nothing is coming out. They seem not to know such thing. If there is any format pliz post me.
