13 May 2013 what is the procedure for applying for duplicate PAN in case it has been lost? Is copy of FIR mandatory? How much does it take to get the duplicate PAN card?

15 May 2013 Form named as 'Request for new PAN card or/ and changes or correction in PAN data' may be filed. FIR is not required. UTIITSL says 15 working days time to issue PAN card.
