One of my friend have to open a pan card but the problem is after her marriage her name is changed and for present name she not have any birth proof document, so please give me information about it.

you can file an affidavit declaring the date of birth and mentioning the fact of non existence of any evidence available for same.

Affidavit as suggested by Sampat Ji will not be accepted since that is not a document as per rule 114(4) of the Income tax Rules,1962.
Please try to obtain a certificate in the format given in link and attach with form 49A.

sir i have done it for my mother and my mother's date of birth have been altered and can show u the affidavit and also the 2 different pan with same number but different date of birth as approved by pan dept.
